On the day of your appointment, we will require you to fill out an online ‘Client Screening Questionnaire’ no more than 2 hours before your appointment. This form ensures you are an appropriate candidate for in-person care, and are comfortable with the risks associated with an in-person appointment. You will not be able to enter the facility until this form is completed. Please note that you will need to repeat this form before each consecutive appointment. If you do not see this form in your inbox the day of your appointment, please check your junk folder and contact our clinic if it’s still missing. There is another form to fill out, consent for treatment in Covid-19 explaining the risks of having treatment during this time of the pandemic.
All clients who enter our space are required to be wearing a mask. You may bring your own mask with you, which can be either cloth or paper. A buff or bandana covering your face is also acceptable. If you do not have a mask, you may purchase one upon your arrival at the clinic for $1. When you enter the clinic, please ensure that your mask is on. If you are purchasing one from us, please indicate this to the receptionist when they call you in for your appointment so that they may have it ready for you.
No one will be allowed to accompany you to your appointment unless you are under the age of 16 and require a parent/guardian, OR you would typically have an assistant with you for safety or mobility concerns. If someone will be accompanying you to your appointment, please let our administrative team know in advance so that we can ensure additional forms are completed by these individuals.
When you arrive at the clinic, please wait outside the clinic or in your vehicle until the time of your appointment. Our front desk staff will call you when they are ready for you to come in for your appointment. Please be sure to provide our administrative team with your cell phone number at the time of booking.
Once you enter the clinic, you must stop at the hand sanitizing station and sanitize your hands. Our receptionist must observe you complete this task, so please ensure she is available and watching before continuing. Please take off your shoes and wear the available slipper. All slippers are thoroughly sanitized after each clients use
Please leave all personal items (including jackets, water bottles and bags) in your vehicle. If you will be arriving by bicycle, we ask that you lock your helmet to your bike, and leave any other non-essential personal items at home.
Any prescribed follow-up appointments will be booked before you leave in the reception area. Once the appointment is complete, you will exit the room. specific circumstances will clients be using our gym space. This may mean that certain equipment will be brought into the office for your use during the appointment. All
equipment is thoroughly sanitized after each client’s use.
After entering the clinic and sanitizing your hands, please sit on the chair as you wait for your physiotherapist to come and get you. Please avoid touching any surfaces while you are waiting for your therapist. When they are available, your therapist will bring you to their room.. Your physiotherapist will also be wearing a mask.
All billing and payments will be processed with direct billing with your insurance company. If you need to pay the balance we have non-contact machine to use for payment.
You will stand on the front of the receptionist who will be behind a plexi-glass partition. The receptionist will confirm that any payment or insurance considerations are in order, then give you the go-ahead to leave the clinic. Your exercise program will also be emailed to you following your appointment.

In the March 23, 2020 statement from the government, they suggested that Physiotherapists can continue to provide “emergency care “only. At this time we cannot provide treatment until the Ontario government and Public Health Ontario advises that it is safe to resume care. At this moment we provide free video call assessment with our clients to help and guide them on what to do in this situation that they can’t access outpatient Physiotherapy.
